Important

1. We schedule new clients for recurring weekly appointments. Treatment time is 12-18 months. Please select a day/time you can commit to on a weekly basis.

2. Choose only one day/time slot. Multiple sign ups from the same person will be deleted.

3. All clients must complete our intake forms in order to reserve their spot.

4. Intake forms will be sent to you by clinic admin after signing up below.
